PREEMPLOYMENT STATEMENT - (Please read carefully and sign the statement below.) I understand and agree that: 1. The information that I have provided on this application is true and complete to the best of my knowledge. Any misrepresentation or omission of any fact in my application, resume, or any other materials, or during any interviews, can be justification for not being offered employment, or for termination from Adam Evans Waterproofing Co., Inc.'s employ if discovered at a later date. 2. In processing my application for employment, the company may verify all the information provided by me, or may procure or have prepared a consumer or an investigative consumer report for this purpose concerning my prior employment, military record, education, character, general reputation, personal characteristics, criminal record, and mode of living. I understand that upon written request to the company, I will be informed whether an investigative consumer report was requested and given full information as to the nature and scope of this investigation. 3. I agree to submit to a pre-employment drug screening by a facility or person designated by the Company and also agree to submit to further examinations and testing as the Company may require if employment is offered. Field employees at Adam Evans Waterproofing Co., Inc. must be able to climb ladders, scaffolding, work from aerial equipment, and must be able to work at heights. They must be able to move rigging and lift 50+ pounds. They must be able to work an eight-hour day on a job site, performing a variety of duties that might include, but is not limited to: applying, installing or performing below grade waterproofing, dampproofing, water repellent, masonry cleaning, joint prep, epoxy flooring. They must be able to handle acids, alkalies, and other chemicals in a safe manner. I understand the job requirements to be employed at Adam Evans Waterproofing Co., Inc. and I certify by my signature below that I am able to perform those requirements. 6. In consideration of my employment, I agree to comply with the policies, rules, regulations, and procedures of the Company and understand that my employment and compensation can be terminated with or without cause or notice, at any time, at the option of either the Company or myself. 7. I acknowledge that the Company reserves the right to inspect all property (including cars, purses, lockers, desks, lunch boxes, packages and other containers) on the Company's premises and job sites and, if employed, I agree to cooperate with such inspections as a condition of continued employment. 8. I understand that this application for potential employment will be given active consideration for only 30 calendar days and that I may thereafter apply again if I wish to receive continued consideration.
